Beyond Estrogen: Understanding Progesterone and Why Balance Matters During Perimenopause

Introduction

When we talk about perimenopause and menopause, estrogen often receives most of the attention.


Yet many symptoms experienced during perimenopause may be connected to another important hormone — progesterone.


In fact, progesterone is often the first hormone to decline as ovulation becomes less regular. Changes in progesterone may influence sleep, mood, stress resilience, inflammation and overall wellbeing.


Understanding progesterone helps us see hormonal changes not simply as isolated symptoms, but as part of a much bigger picture.

 

Progesterone: More Than a Reproductive Hormone

Progesterone is often thought of as a fertility hormone, but its role extends far beyond reproduction.

It is involved in:

  • sleep quality
  • nervous system balance
  • emotional wellbeing
  • inflammatory responses
  • menstrual balance
  • stress resilience

 

This is one reason why many women notice anxiety, poor sleep, PMS, heavier periods or increased sensitivity during perimenopause, even before menopause itself.


Perimenopause Is Often a Progesterone Story

Unlike menopause, where hormone levels are consistently low, perimenopause is characterised by fluctuations.


Ovulation may become irregular, meaning progesterone production can vary greatly from cycle to cycle.


This can contribute to:

  • insomnia
  • anxiety
  • irritability
  • breast tenderness
  • bloating
  • heavier periods
  • increased stress sensitivity

While estrogen is often discussed, progesterone changes may sometimes play an equally important role.

 

Stress, Blood Sugar and Hormonal Balance

Hormones do not work in isolation.


Sleep, stress, inflammation and blood sugar balance all influence one another.


Chronic stress and fluctuating blood sugar levels may increase cortisol demands, affecting how the body adapts during perimenopause.


Recently, the term insulin resistance has become more widely discussed. It is thought to be associated with stress, poor sleep, visceral fat accumulation and hormonal changes during midlife.


Over the past year, after personally stopping alcohol, I noticed fewer late-night cravings and improvements in sleep quality. Experiences like these have made me increasingly aware of how deeply interconnected these systems appear to be.

 

New Concepts: Progesterone Resistance and Sensitivity

Emerging areas of functional medicine are beginning to explore concepts such as progesterone resistance and progesterone sensitivity.


Some women may experience symptoms associated with low progesterone even when hormone levels appear adequate, while others may be particularly sensitive to progesterone therapy itself.


Research in these areas is still evolving, and these concepts are not yet part of conventional medical diagnosis. However, they highlight how individual hormonal responses can vary greatly from person to person.


Inflammation, stress, digestive health and histamine pathways are all thought to influence hormonal balance, reminding us that hormones rarely act alone.



Aromatherapy and Whole-Body Support

Aromatherapy does not aim to replace hormones or “fix” the body.


Instead, it offers gentle support for the whole person during times of hormonal change. In clinical aromatherapy, blends are rarely created for one symptom alone. Rather, they are designed to support the interconnected systems of the body.


Essential oils traditionally used during perimenopause and menopause may be chosen to support:

  • Stress and nervous system balance
    Examples: Lavender, Roman Chamomile, Sweet Marjoram and Frankincense.
  • Sleep and emotional wellbeing
    Examples: Lavender, Bergamot, Roman Chamomile and Sandalwood.
  • Inflammatory responses and physical comfort
    Examples: Frankincense, Helichrysum, German Chamomile, Blue Tansy, Black Spruce.
    Some of these oils contain constituents that have attracted scientific interest for their anti-inflammatory properties, while azulene-rich oils such as German Chamomile and Blue Tansy are traditionally valued for their soothing qualities and have demonstrated antihistamine activity in laboratory studies.
  • Digestion and gut function
    Examples: Cardamom, Peppermint and Ginger.
  • Circulation and muscle comfort
    Examples: Ginger, Sweet Marjoram, Rosemary and Black Spruce.
  • Respiratory and immune support
    Examples: Eucalyptus radiata, Ravintsara and Niaouli.
  • Mood and emotional uplift
    Examples: Sweet Orange, Grapefruit, Bergamot and Mandarin.

Individual essential oils often possess several therapeutic properties at the same time. For example, one oil may support emotional wellbeing while also offering benefits for physical comfort, digestion or circulation.


This complexity is one of the reasons I enjoy blending. By combining carefully selected oils, it is possible to create blends that support several aspects of wellbeing simultaneously, rather than focusing on one symptom alone.


This is also one of the strengths of bespoke aromatherapy. Rather than taking a “one blend fits all” approach, blends can be tailored to the individual, recognising that stress, sleep, digestion, inflammation and emotions are all closely connected.


This holistic approach is one reason why aromatherapy rarely focuses on one hormone or one symptom. Instead, it aims to support balance throughout the body and mind, recognising how closely these systems are connected.

The more I learn about hormones, the more I realise that perimenopause is not simply an estrogen story.


Progesterone, sleep, stress, inflammation and blood sugar regulation all appear deeply interconnected.


Perhaps wellbeing during this stage is not about “fixing” one hormone, but about supporting the body as a whole with gentleness, patience and understanding.
